In this presentation, Dr. Urban Emmenegger explores the benefits and limitations of treatment intensification for prostate cancer, comparing hormone therapy alone with combination approaches such as androgen receptor inhibitors, chemotherapy, and other treatment strategies. It reviews the key factors that influence treatment decisions, including survival outcomes, quality of life, monitoring requirements, cost and insurance considerations, and individual patient preferences. The presentation also discusses how clinicians tailor treatment recommendations based on disease characteristics, such as tumor burden, symptoms, PSA levels, and imaging findings, alongside patient-specific factors including age, overall health, and personal values.

This presentation reviews the current pathway for diagnosing prostate cancer and examines evidence supporting a newer, faster diagnostic approach. It also highlights outcomes of ablative therapies, including irreversible electroporation (IRE) and stereotactic ablative radiotherapy (SABR), and presents results from 5,000 transperineal prostate biopsies performed at North York General Hospital (presented by Barbod Azimikhiabani and Veselina Stefanova).

Dr. Geoffrey Gotto, Urologic Oncologist at the Southern Alberta Institute of Urology, joins us again to explain PARP inhibitors and their importance in modern treatment of cancer cells. By blocking DNA repair, the inhibitors help kill off cancer cells to help in treating the disease. Dr. Gotto discusses how to make inhibitors more effective, how to pair them with additional treatments, and how genetic testing is helping doctors identify cancers sooner and with greater precision.

Jonathon Gabbai, MBA, BSc.PT Senior Consultant, Provincial Pathways Unit, Primary Care Alberta joins us to discuss how his provincial health team is helping support better, more consistent care in Alberta for those affected with prostate cancer.
